- Required Certifications: BLS, MSW
- Required Skills: Biopsychosocial assessment, Child protective services, Collaborative care, Collect and analyze patient information, Conduct initial evaluation, Coordinate services to home care and equipment following discharge, Coordinates with internal and external team members, Discharge Planning, Documentation according to plan of care, Drug/alcohol use and treatment options, Emergency detention, Facilitate discharge and contingency planning, Guardianship, Management/lead experience, Medicaid, Medicare, Adult protective services, Case management, Develop plan of care to meet patient/caregiver needs, Discharge Planning, Discharge Summary, Documentation reflects understanding of coverage criteria, Documents patient response, Documents pertinent information to facilitate care coordination, Facilitate social and psychological well being of nursing home residents, HMOs, Knowledge of cultural issues, Medical Transportation, NASW Code of Ethics/Standards of Home Health Care, Plan of Care, Review of Health history, Participates in care conference activities, Plan of Care, Power of Attorney, Refers to other disciplines/resources, Refers to rehab and long term care facilities, Social Security
